Hi Benjamin
Try to do this configuration in your IVR Survey,
Open your IVR Survey Opt-In, In Survey IVR Settings configure the field like the image below. (Give your own name to your script to show it on Report).
Now, In your Survey...Open the Survey IVR Setting and only filled the field "Script Name for Monitoring and Reporting" like the image below
Now make a call to test your configuration is OK, and you can see the result in Online Monitoring and Reports.
